Consider referral for patients with
- First-time seizures or suspected epilepsy
- Recurrent blackouts or loss of awareness
- Unexplained confusion, staring episodes, or cognitive changes
- Sleep-related neurological events
- Possible psychogenic non-epileptic seizures (PNES)
- Suspected non-convulsive seizures or altered mental status

A normal routine EEG does not exclude epilepsy. Patients with epilepsy may have normal recordings between seizures. Sleep EEG, ambulatory EEG, or prolonged monitoring may be appropriate depending on the clinical picture.
